Endoscopic submucosal dissection using a novel irrigation hood-knife.

چکیده
Endoscopic submucosal dissection tech− niques using an insulated−tip knife (IT knife) were introduced several years ago to allow en−bloc resection of early gastric cancers, since histological evaluations are essential in order to assess the risk of lymph−node metastasis [1, 2]. However, the technique requires special skills and involves a prolonged procedural time. We have therefore developed a new type of dissection knife, termed the “hood−knife” (patent pending in Japan), which only re− quires a simple technique.

Endoscopic submucosal dissection of a nonpolypoid superficial neoplasm of the terminal ileum.
A 73-year-old man underwent colonoscopy for recurrent diffuse abdominal pain. Previous colonoscopy, routine laboratory tests, and a fecal occult blood test had all shown normal results. At retrograde ileoscopy, a 25-mm laterally spreading tumor, nongranular pseudodepressed type (LST-NG PD; type 0-IIa-IIc according to the Paris classification), was diagnosed 5cm proximal to the ileocecal valve (...
متن کاملEfficacy of a Novel Narrow Knife with Water Jet Function for Colorectal Endoscopic Submucosal Dissection
BACKGROUNDS With respect to the knife's design in colorectal endoscopic submucosal dissection (ESD), diameter, water jet function, and electric power are important because these relate to efficient dissection. In this study, we analyzed a novel, narrow ball tip-typed ESD knife with water jet function (Flush knife BT-S, diameter: 2.2 mm, length: 2000 mm, Fujifilm Co., Tokyo, Japan) compared to a...
متن کاملPolyethylene glycol submucosal irrigation: a novel approach to improve visibility during endoscopic submucosal dissection
In order to expand the availability of endoscopic submucosal dissection (ESD), measures to facilitate the procedure are necessary. When bleeding occurs, the examiner's field of vision is critically impaired, and ESD becomes less efficient and more hazardous because of the presence of submucosal hematoma and covered blood clot. We propose the use of polyethylene glycol (PEG) irrigation as a simp...
متن کاملEndoscopic Submucosal Dissection (ESD) in Colorectal Tumors
BACKGROUND Endoscopic submucosal dissection (ESD) is accepted as a minimally invasive treatment for early gastric cancer, however, it is not widely used in the colorectum because of its technical difficulty. OBJECTIVE To determine the feasibility of using ESD for treating large superficial colorectal tumors. PATIENTS A total of 400 consecutive patients were treated by ESD for 405 lesions at...
متن کاملTwo electrosurgical endo-knives for endoscopic submucosal dissection of colorectal superficial neoplasms: a prospective randomized study
BACKGROUND AND STUDY AIMS Few studies have directly compared endo-knives for endoscopic submucosal dissection (ESD) in humans. We compared the performances of the Mucosectom2 and SB knife Jr. PATIENTS AND METHODS Two trainee endoscopists performed ESD of 36 lesions in this prospective, randomized controlled trial. Mucosal incision with a 1.5-mm Dual knife and submucosal dissection using the...
